// This study investigated speech-in-noise (SiN) perception difficulties in cystic fibrosis patients treated with aminoglycosides, focusing on cochlear and neural damage across extended-high-frequency (EHF) and standard frequency (SF) ranges. Using audiometry, otoacoustic emissions, and middle ear muscle reflex tests on 101 CF patients and 84 controls, they found that SF hearing loss, not EHF loss, mainly predicted poorer SiN performance, with increased efferent neural activity also linked to difficulties. The findings highlight combined sensory and neural damage as key contributors to SiN challenges in CF, suggesting prevention and intervention should address both mechanisms.
