// This study analyzed gender and racial diversity trends in US neurotology fellowships from 2007 to 2024 and compared them with otolaryngology residency trends. It found that while female and racial minority representation in neurotology fellowships increased, gains were slower and less pronounced than in residencies. Black and American Indian/Alaska Native trainee representation remained very low in fellowships. The results suggest structural barriers limit diversity progression to subspecialty training, highlighting the need for targeted recruitment efforts.
