// This study examined how obstructive sleep apnea (OSA) severity leads to postoperative complications in high-risk pediatric patients undergoing adenotonsillectomy. Using causal mediation analysis on 307 patients, it identified oxygen nadir, maximum CO2, and time with oxygen saturation below 90% as key mediators of complications like ICU intervention and prolonged hospitalization. The novel finding is that time with low oxygen saturation is an important marker beyond traditional OSA severity scores. However, these mediation effects are sensitive to unmeasured confounding, so results should be interpreted cautiously.
