// The study tested whether military Veterans with normal audiograms but constant tinnitus and/or decreased sound tolerance (DST) show physiological signs of cochlear deafferentation compared with young, low-noise-exposure controls. Researchers measured ABR wave I amplitude, envelope following response (EFR) magnitude, wideband middle ear muscle reflex (MEMR) magnitude, and high-frequency DPOAEs. Veterans had reduced ABR wave I amplitudes, smaller EFRs, and diminished high-frequency DPOAEs (with average MEMR reductions in the DST group but inconsistent MEMR findings in tinnitus cases) and often elevated ABR V/I ratios. The results suggest cochlear deafferentation and subclinical high-frequency outer hair cell dysfunction in these Veterans, MEMR is an unreliable deafferentation marker, and increased central gain may contribute to tinnitus and DST.
