// This study tested whether middle-aged adults with tinnitus but clinically normal hearing show peripheral neural dysfunction and duration-related brainstem changes. Click-evoked auditory brainstem responses (45–90 dB nHL) were recorded in 74 tinnitus patients (acute <6 months, chronic ≥6 months) and 15 controls, analyzing wave latencies, amplitudes, interpeak latencies, intensity slopes, and V/I ratios. Pure-tone thresholds and DPOAEs did not differ, but chronic tinnitus showed prolonged wave I latency at 55, 75, and 80 dB and both acute and chronic tinnitus had reduced wave I amplitude at 45 dB; chronic cases also showed a steeper wave V latency-intensity slope and higher V/I ratio at 80 dB. The findings point to selective auditory nerve (wave I) abnormalities in normal-audiogram tinnitus with additional suprathreshold brainstem changes in chronic tinnitus.
