// Retrospective cohort study of 470 adults with vestibular migraine (VM) or probable VM (PVM) (01/2024–12/2025) assessed demographics, audiometry, videonystagmography (VNG) and VHIT to determine prevalence of hearing loss and cochleovestibular paresis. Sixty-one percent underwent audiometry and 40% of those tested had hearing loss (mostly mild–moderate; 51% unilateral), 6.6% had documented cochleovestibular paresis, VNG was abnormal in 88% (commonly central positional nystagmus) and VHIT was abnormal in 56%. There were no significant differences between VM and PVM in prevalence or severity of hearing loss or cochleovestibular paresis. Findings support a single pathophysiological continuum for VM and PVM, with family history of migraine (64% VM vs 49% PVM) the main clinical differentiator.
