// This study tested intrarater reliability of nasopharyngoscopy ratings of velopharyngeal closure by comparing clinical reports to standardized research re-ratings. Five speech-language pathologists rated closure pattern, percent closure, and velar movement; reliability was quantified with Cohen’s kappa and ICC for clinical vs research re-ratings and for research re-ratings repeated. Reliability between clinical and research ratings varied widely (k = .24–1.00; ICC = −.09–.81), whereas within the research setting reliability was consistently higher (k = .68–1.00; ICC = .70–.99). The authors conclude that clinical ratings may not generalize to research unless raters use standardized definitions and consistent training, which can achieve high reliability.
