// This review synthesizes decades of translational research testing respiratory-swallow training (RST) as a mechanism-based intervention to improve swallowing safety and physiology in people treated for head and neck cancer. It integrates experimental, observational, and interventional studies using synchronized videofluoroscopy with respiratory measurement, Phase I/II clinical trials and ongoing randomized trials, and work on wearable sensors and telehealth for home training. Findings show safe swallowing is linked to expiratory-phase (mid–low tidal volume) swallow initiation, RST reliably shifts swallows into that phase and yields significant reductions in physiologic impairment and penetration-aspiration with better airway protection, bolus clearance, intake, and patient-reported outcomes. These results support respiratory-swallow coordination as a modifiable target and RST as a mechanism-driven, scalable approach for dysphagia rehabilitation.
