// This retrospective study of 42 adult cochlear implant recipients used pre- and postoperative imaging and linear mixed-effects models to test whether scala tympani (ST) volume, cochlear duct length, cross-sectional measures, or angular insertion depth predict low-frequency hearing change. ST volume and biological sex significantly predicted postoperative low-frequency threshold shift (P = 0.005 and P = 0.01), while CDL, AID, and local ST cross-sectional measures did not; age showed a trend. The authors conclude overall ST volume more strongly affects hearing preservation than local electrode–tissue geometry, suggesting indirect trauma mechanisms (e.g., hydraulic pressure during insertion) may predominate. They recommend validating this by measuring intraoperative fluid pressure transients relative to ST volume.
