// This study tested whether tympanic membrane signs and hearing tests can distinguish otitis media with ANCA-associated vasculitis (OMAAV) from refractory otitis media with effusion (OME). In a retrospective cohort (21 OMAAV, 9 OME) the authors scored tympanic membranes with the SCOT system and performed frequency-specific pure-tone audiometry, then used ROC and logistic regression analyses. OMAAV cases had higher SCOT scores (notably vascular dilatation and posterior wall swelling) and worse air conduction at 2000/8000 Hz and bone conduction at 250/500 Hz. Combining the SCOT total score with bone conduction at 500 Hz gave AUC 0.952 (sensitivity 100%, specificity 88.9%), suggesting this combo can flag patients for further OMAAV workup including ANCA testing.
