// We prospectively studied 392 infants born ≤32 weeks GA who had diffusion MRI at term-corrected age with automated tractography of the corpus callosum and Bayley-III testing at 2 years, using multivariable regression and mediation analyses to link bronchopulmonary dysplasia (BPD) severity, callosal fractional anisotropy (FA), and development. Higher BPD grades were associated with lower FA in the posterior midbody and isthmus of the corpus callosum. Greater BPD severity was also linked to lower Bayley-III cognitive, language, and motor scores. Callosal microstructure modestly mediated the cognitive association (posterior midbody explained 12.7% of the effect), but most mediation effects were small/non-significant and a direct adverse effect of BPD persisted.
