// This study tested whether juvenile recurrent parotitis (JRP) is associated with later autoimmune disease and whether associations differ by sex and age. Using a Danish nationwide cohort (1994–2024) of 2,915 JRP cases and 29,140 matched controls, cumulative risks were estimated with the Aalen–Johansen estimator and hazard ratios with Cox regression across ten autoimmune groups. JRP was linked to higher autoimmune risk (cumulative 4.73% vs 3.02%; HR 1.71), notably Sjögren’s disease (HR 28.16), connective tissue diseases (HR 3.69), and gastrointestinal diseases (HR 1.55). Associations were stronger in females and adolescents, suggesting JRP may signal increased risk or an early indicator of autoimmune disease.
