// This systematic review (MEDLINE, Embase, Scopus to June 2025, PRISMA) sought and appraised studies of interventions for primary pediatric tracheomalacia. Twenty-two mostly retrospective studies (764 children) were included across surgical pexy, airway stenting, external stabilization, and medical management. Surgical pexy was the most studied and showed clinician-reported resolution in 73–100% with sustained benefit on available follow-up, whereas stents provided immediate patency but poor long-term durability. All evidence comes from uncontrolled observational studies, so prospective registries, comparative studies with non-surgical controls, and routine patient-reported outcomes are needed.
