// This review examines whether spiral ganglion neuron (SGN) integrity predicts cochlear implant outcomes and assesses the feasibility of otic neural progenitor (ONP) cell therapy by surveying electrophysiological diagnostics and surgical delivery approaches. Cadaveric and preclinical studies show surgical delivery is technically feasible, and the evidence suggests neural function rather than absolute SGN counts often limits implant benefit. Current electrophysiological tools are indirect and cannot identify SGN degeneration in individuals with the specificity required for trial enrollment. ONP therapy is promising but still investigational and will require rigorous patient stratification, validated diagnostic and outcome measures, and demonstration of long-term safety, cell survival, differentiation, connectivity, and functional integration with implants.
