// This multi-institutional retrospective cohort of 23 patients evaluated neoadjuvant immune checkpoint inhibitor (IO) therapy for temporal bone–involving squamous cell carcinoma to inform neurotologic decision-making. Patients received neoadjuvant IO with radiologic (RECIST 1.1) and pathologic response assessment and subsequent surgical management as indicated. Overall 26% achieved complete response and 17% partial response (≈43% total responders), with higher response in cutaneous primaries (≈67%); about 9% avoided surgery and another 9% had less extensive operations. Responders had better 3‑year survival (100% vs 78% for progressive disease, P=0.029), suggesting neoadjuvant IO is a feasible option that can permit surgical de‑escalation in selected patients pending phase 3 trial data.
