// This retrospective single-center cohort study compared retrosigmoid vestibular schwannoma resection with an operating microscope (n=103) versus an exoscope (n=142) using identical techniques and neuromonitoring. Operative time, extent of resection, facial and cochlear nerve preservation, CSF leak, and revision rates were similar between groups. At ≥1-year follow-up the exoscope group had significantly better facial nerve function (lower House-Brackmann scores; adjusted OR 2.54, P=0.006). The authors conclude exoscope-assisted surgery matches microscope outcomes without longer operative times and adds ergonomic and educational benefits.
