// This prospective observational study used continuous overnight pulse oximetry to measure clinically significant desaturation (SpO2 <85% for >30 s) or need for supplemental oxygen in 147 non-syndromic children admitted after adenotonsillectomy. Thirteen children (8.8%) had ≥1 severe desaturation and 11 (7.5%) required overnight supplemental oxygen; those with severe events had more frequent and longer moderate desaturations. Need for supplemental oxygen in the PACU strongly predicted overnight oxygen requirement (OR 12.1, 95% CI 4.3–34.3; p<0.001). The authors conclude severe overnight desaturation is uncommon but identifiable and suggest considering overnight observation for patients with preoperative URI symptoms or PACU oxygen needs, with further study of predictors warranted.
