// This retrospective study evaluated inter- and intrarater reliability of the Symons and Fanning CT classification in 63 surgically confirmed otosclerotic ears, using independent assessments by a neuroradiologist and an ENT specialist on preoperative cone-beam CT and correlating ratings with hearing level, round-window involvement, and stapes footplate thickness. Interrater reliability was weak (with only a slight increase after six months), and SFC grade showed no correlation with hearing level, round-window involvement (16% prevalence), or stapes footplate thickness. Therefore the SFC on CBCT demonstrated limited reproducibility across specialties and only conditional predictive value for preoperative audiologic counseling.
