// This retrospective cohort study examined how preoperative audiometric measures predict post-cochlear implant hearing outcomes to improve counseling beyond binary candidacy. Researchers analyzed preoperative pure-tone average (PTA) and word recognition score (WRS) in both implanted and contralateral ears and tracked postoperative WRS to ~6 months, defining success as WRS≥60% or a clinically meaningful ΔWRS≥20%. Implanted-ear status showed slightly better overall predictive performance, but age modified relationships: patients <60 years improved faster when the implanted ear had residual hearing (pre-PTA<60 dB), while patients ≥60 years depended on a good contralateral WRS (≥60%) to improve. These age-stratified patterns enable more personalized prognostic counseling on time to functional hearing gains after implantation.
