// This pilot randomized trial tested whether daily donepezil (5 mg) for 90 days improves speech perception and cortical plasticity in adult cochlear implant users versus placebo. Donepezil recipients showed a mean 14.5-point improvement in implanted-ear CNC word recognition (vs −0.8 points for placebo), plus improvements in binaural and sentence-in-noise measures and reduced listening fatigue. Functional near-infrared spectroscopy showed increased left frontal activation with donepezil, supporting cholinergic-induced neuroplasticity, but the study was small and underpowered, so larger trials are needed.
