// This systematic review examined whether adding obliteration to canal wall up (CWU) tympanomastoidectomy affects clinical, patient-reported, and economic outcomes for adult cholesteatoma. The authors performed a comprehensive search through July 7, 2025 and included mostly retrospective studies (four comparative and five noncomparative) comparing CWU with and without obliteration; no prospective trials, randomized studies, or economic evaluations were found. Most included studies reported lower recurrent/residual disease (reduced recidivism) with obliteration added to CWU. However, the evidence is low quality, heterogeneous, and likely confounded, so definitive conclusions cannot be drawn.
