// This study used cadaver dissection, 3D anatomic modeling, and perilymph sampling in cadavers and five surgical patients to identify the head position that maximizes intratympanic dexamethasone delivery through the round window compared with the traditional 45° yaw. The optimal orientation was 58.9° yaw, 31.1° roll, and 22.5° pitch; 3D modeling showed roll and pitch were critical to direct fluid toward the round window, and pooled cadaveric/clinical samples trended toward 2.94-fold higher perilymph dexamethasone in the ideal position (0.0447% vs. 0.0152%), though this did not reach statistical significance. Implementing this simple 3-axis head adjustment (yaw+roll+pitch) may cost‑effectively improve intratympanic steroid absorption and therapeutic efficacy.
